When is upgrade required?

Upgrade your code from the dashboard when the status is "Upgrade Required"

An upgrade is only required when you change the safe page URL,not for any other configuration like filter or condition update.

It is also required when a new version of the code is available which includes the new improvements.

If the upgrade fails from the dashboard, ensure that you've setup correct permissions and if it still fails then download the new code and replace the existing code manually using SFTP file upload.
